Each Pear Safety Alert is written in our own words from primary documents we archive and cite by number.
They are free to download, brief, and reuse in your own toolbox talks. The primary alerts remain
authoritative and are available from the issuing bodies.

Lifting integrity: four documents, one lesson, ten weeks

Between June and August 2026, four documents under three letterheads described the same family of
lifting failure: inspections that could not see the failing component, OEM instructions carried as
advice rather than requirement, and people in the line of fire when the gear let go. Three are
individual alerts. The fourth is a regulator counting five years of them. Read side by side, they
carry one lesson and five actions.

How these exist
These alerts exist because the primary documents were filed in one register and read side by side.
That is the whole method, and it is available to any safety team willing to keep its records where
they can meet.
